Hayden Panettiere’s cause of death is finally official. More than a month after the 36-year-old actress died in Greenville, South Carolina, the county coroner’s office announced Tuesday that fentanyl and several other drugs caused her death. Officials ruled the death an accident.
On August 16, a 911 caller reported Panettiere in cardiac arrest at a residence where she was temporarily staying. First responders attempted advanced life support. Paramedics pronounced her dead at 2:32 p.m.

According to the coroner’s official statement, the cause of death was “toxic effects of fentanyl, 4-ANPP, alprazolam, methocarbamol and quetiapine.” A board-certified forensic pathologist performed the autopsy and found no signs of trauma. Parade reported that the coroner’s office confirmed the findings to Variety on September 22.
For readers unfamiliar with the substances named in the report, here is a quick breakdown. 4-ANPP is a chemical precursor used in illicit fentanyl production. Alprazolam is better known by its brand name, Xanax. Methocarbamol is a prescription muscle relaxant, and quetiapine is an antipsychotic medication. The Gray Television report listed the same substances after speaking with the coroner’s office.
Most importantly, officials classified the manner of death as an accident. Police had previously said the early investigation showed no signs of foul play or suspicious circumstances.
What Happened on August 16
Panettiere was found unresponsive by her on-again, off-again boyfriend, Brian Hickerson, and his brother, Zach Hickerson. The two men were with her at the home that afternoon. Reports said those present administered Narcan and performed CPR in an attempt to revive her before paramedics arrived. A police report noted that Brian directed officers to a bag containing medication Panettiere had been taking.
